If you have an illness lasting seven days or less you do not require a doctor’s sickness certificate. Your employer may require you to complete a self-certification form called an SC2 which is available from the HMRC website.
Fit note (Formally known as sick certificate or MED3)
The Surgery requests that all patients follow the procedure set out below when requesting a fit note.
1st Fit Note following surgery or an inpatient stay at hospital:
Please request in writing and allow 72 hours for processing. The GP will need a copy of any supporting documents such as a discharge summary or post-operative discharge.
Fit Note Extension
Please request in writing and allow 72 hours for processing. If the GP needs to see you they will contact you to arrange an appointment at the Surgery.
Non-urgent advice: Patient Notice
Please do not make an appointment for a certificate.
